Required clinical activity in the last 24 months (case logs)- Applicant must provide documentation of provision of family medicine services representative of the scope and complexity of the privileges requested during the previous 24 months (applicants who have completed family medicine training during the previous year must provide a training log). Will provider work solo? If not, other providers specialties working with them – Yes, can work solo Licensed Candidate: Yes/No – Yes, must hold active license at time of present – Must have OR license not willing to license Note: will not accept candidates that do not have an active license in Oregon but will consider applicants that have a license pending depending on what the expected license issue date is.
