Required clinical activity in the last 24 months (case logs) Applicant must provide documentation of provision of cardiovascular disease services representative of the scope and complexity of the privileges requested during the previous 24 months. (waived for applicants who completed training during the previous year) Will provider work solo? Yes, there are no other cardiologist here on island. Will be supported by family medicine, OBGYN, Pediatrics, Orthopedics, General Surgery, ED, hospitalist Licensed Candidate: Yes, active AK license in hand at time of present Required certifications (Must be from AHA or American Red Cross as applicable) BLS EMR EPIC Proficient user
